Description

In Delivery Outcomes, when Death is selected as the outcome, the form does not get submitted even after filling all required fields.

This issue blocks completion of delivery outcome records for death cases.

Steps to Reproduce

1. Open FLW application

1. Navigate to Delivery Outcomes

1. Fill all mandatory delivery details

1. Select Death as the outcome

1. Click on Submit

Actual Result

• Form is not submitted

• No success message is shown

• User remains on the same screen

Expected Result

• Form should submit successfully when Death is selected

• Proper confirmation message should be displayed
