1. Are you using any Family Planning Method?: Yes / No

If "Yes" is selected show Methods in dropdown and (i.e. if "Yes" is selected don't show below 2nd question)

If "No" is selected, then show below question:

2. Do you want to use any Planning Method? Yes / No

If "Yes" is selected show Methods in dropdown

If "No" is selected, end and submit

Rename: "Method of Contraception" to "FP Methods" it applicable to all App
