      As an ASHA, I want to record malaria slide collection details for beneficiaries with fever symptoms, so that malaria cases can be diagnosed, tracked, and reported timely under the NVBDCP program.

      Acceptance Criteria

      Beneficiary Identification

      • In the Beneficiary list, ASHA can view and filter individuals who have reported fever symptoms during household visits.
      • Filter option “Fever Cases” should be available in the Beneficiary section.
      • Only Active (Alive) beneficiaries are shown for slide collection.

      Malaria Slide Collection Form

      When ASHA selects a beneficiary and clicks “Add Malaria Slide Collection”, the following form should appear:

      Field Description / Validation
      Date of Visit Default: NULL; Cannot be greater than today’s date
      Fever Duration Free text or dropdown (e.g., <2 days, 2–5 days, >5 days)
      Temperature Recorded (°C) Optional numeric field; must be between 35–45°C
      Symptoms Observed Multi-select: Fever with chills, Headache, Sweating, Nausea/Vomiting, Others
      Slide Collected Yes / No
      Date of Slide Collection Mandatory if “Slide Collected” = Yes; Cannot be future date
      Slide ID / Barcode Text field (auto or manual entry)
      Slide Sent To Lab Yes / No
      Date Sent to Lab Mandatory if “Sent to Lab” = Yes; Cannot be future date
      Lab Name / Facility Dropdown or free text
      Result Received Positive / Negative / Pending
      Result Date Mandatory if “Result Received” ≠ Pending; Cannot be future date
      Treatment Given Dropdown: ACT, Primaquine, Other / None
      Follow-up Status Recovered / Ongoing / Referred to PHC
      File Upload (Lab Report) Optional; PDF/JPG/PNG

      Form Actions

      • Submit Button to save malaria slide collection details.
      • On successful submission, show message: “Malaria slide collection details saved successfully.”
      • Records should be available under View/Edit Malaria Slide Collection for updates or verification.
