As an ASHA I want the system to automatically adjust form behavior and guide me to the MDSR (Maternal Death Surveillance and Response) section so that I can efficiently report maternal deaths and avoid filling irrelevant fields.

Enhance the user experience in the ANC, Delivery Outcome, and PNC sections by dynamically disabling/hiding irrelevant fields when a maternal death is recorded, and by seamlessly navigating the ASHA to the MDSR reporting section.

Also, update the file upload icon in ANC and Delivery Outcome sections for clarity.

Acceptance Criteria

PNC Section – Field Reordering and Conditional UI

• Move the field “Is mother dead?” to appear immediately after "PNC Visit Date".
• Set the default value of "Is mother dead?" to “No”.
• If the answer is “Yes”:
  • Automatically hide all fields below "Is mother dead?".
  • On form submission, redirect the user to the MDSR section.

Delivery Outcome – Conditional Navigation and Field Behavior

• In the Delivery Outcome form:
  • If maternal death is recorded:
    • Disable/hide non-relevant fields.
    • On submission, automatically redirect the user to the MDSR section.

ANC Section – Conditional Navigation and Field Behavior

• In ** ANC visit forms:
  • If maternal death is reported:
    • Disable/hide irrelevant fields.
    • On submission, automatically navigate to the MDSR section.

File Upload Icon Update

• Replace the current file upload icon in:
  • ANC section
  • Delivery Outcome section
• The new icon should be:
  • Intuitive (e.g., a document or cloud upload symbol)
  • Consistent with the overall UI theme
  • Mobile-friendly and easily tappable
