As an ASHA, I want to confirm whether a baby has been discharged from the SNCU and upload the discharge summary, so that this information can be auto-populated in the HBNC section and I can claim the related incentive.

Acceptance Criteria

Discharge Status Field:

• A mandatory radio button is displayed with the question: "Is Baby discharged from SNCU?"
  • Options: Yes / No
  • Default selection: No
• The selected value is auto-populated in the HBNC section of the application.

Discharge Summary Upload:

• Field labeled: "Discharge Summary"
• Allows upload of up to 4 images
• ASHA can view uploaded images
• Button/option to update with the latest images

User Interaction:

• If "Yes" is selected, allow image upload or redirect to upload section.
• If "No", allow submission but flag that incentive claim may be incomplete.
