As an ASHA, I want to support the Extended PMSMA (ePMSMA) initiative by mobilizing High-Risk Pregnancies (HRPs) for follow-up ANC check-ups and ensuring healthy outcomes post-delivery, so that I can receive case-based incentives as per MoHFW guidelines.

MoHFW has rolled out Extended PMSMA (ePMSMA), to be implemented across all States/UTs.
Under this initiative:

• PMSMA check-ups are to be conducted on 9th, 10th, and 23rd of every month.
• The focus is on missed out HRPs or those requiring frequent follow-ups.

Acceptance Criteria:

• Incentive for HRP Follow-Up Mobilization:
  • ASHA to receive ₹100 per HRP for each follow-up ANC visit to a PMSMA clinics/nearest facility.
  • Maximum of 3 follow-up visits per HRP eligible for incentive.
  • System should track:
    • HRP identification (flagged case)
    • Date of each follow-up ANC visit
    • Facility where visit occurred
    • Confirmation of ASHA involvement in mobilization
• Incentive for Healthy Outcome Post-Delivery:
  • ASHA to receive ₹500 per HRP if:
    • A healthy outcome (mother and baby) is achieved by the 45th day post-delivery
    • Verified and confirmed by the concerned ANM and MO

• • System should trigger eligibility check after 45 days of delivery for HRP-tagged cases.

• Date & Facility Constraints:
  • The follow-up visits must occur on 9th, 10th, or 23rd of the month.

• • Must be recorded at a PMSMA facility or designated nearest facility.
