As an ASHA, I want to view the incentive count and amount categorized by activity for a selected month and year, so that I can track the number of claims and total incentives received for each activity.

Acceptance Criteria:

• Month/Year Selection Dropdown:
  • User can select the month and year from a dropdown list.
  • On selection and clicking "Show", the system retrieves incentive data for that period.
• Activity-wise Incentive Display:
  • A table displays:
    • Serial Number
    • Activity Name
    • Number of Claims
    • Amount (in Rs.)
  • The data shown is specific to the selected month/year.
• Details Navigation:
• Clicking on any activity row navigates to a detailed view.
  • The detailed view lists:
  • Beneficiary name
  • Beneficiary ID
  • Individual incentive amount
• Language addition (convert form in Assamese/Hindi/English too) as per selection
• Total Incentive Display:
  • A button or label shows the total incentive amount for the selected period.
