Leprosy – “Referred To” Field Shown & Editable for All Statuses Instead of Only “Suspected”, and Follow‑Up Date Should Be Optional

      Description:

      Two issues have been identified in the Leprosy workflow:

      • "Referred To" field – Shown & editable for all Leprosy statuses; should only appear when status = "Suspected".
      • Follow‑Up Date – Currently mandatory; should be optional.

      Steps to Reproduce:

      1. Log into the application and navigate to the Leprosy module.

             2. Add or edit a record and select any Leprosy status other than "Suspected".

             3. The "Referred To" field is still visible and editable.

             4. Change status to "Suspected" and note that the behavior is the same.

             5. Attempt to save the form without entering a Follow‑Up Date — system blocks save.


      Expected Result:

      • "Referred To" field should only be displayed & editable when status = "Suspected".
      • Follow‑Up Date field should be optional and not block submission when left empty.

      Actual Result:

      • "Referred To" is always shown and editable regardless of status.
      • Follow‑Up Date is wrongly enforced as mandatory.
