Description:
In the Malaria submodule, for each symptom where a “Not Applicable” dropdown is present, the options are displayed correctly, but the default "No" option is not pre-selected when the screen is opened. 

Steps to Reproduce:

1. Log into the application and navigate to the Malaria submodule.

       2. Open a symptom entry form that contains the “Not Applicable” dropdown.

       3. Observe that no option is pre-selected on screen load.

Expected Result:

• Upon opening the form, the “No” option should be pre-selected by default for the “Not Applicable” field.

Actual Result:

• Field displays options, but no default selection is applied.
