Malaria – IRS Rounds: Multiple Submissions Allowed Beyond Limit, Missing "History" Heading, and Table Row Spacing Issue

      Description:
      In the Malaria module’s IRS Rounds section, the following issues are observed:

      1. When the IRS Rounds toggle button is turned ON, the system allows more than 4 submissions, whereas the maximum should be restricted to 4.

             2. The "History" heading is missing in the round table, reducing clarity for users.   

             3. There is no spacing between rows in the round table, causing a visually cluttered interface and making it difficult to read.

      Steps to Reproduce:

      1. Log in to the application and navigate to Malaria → IRS Rounds.

             2. Toggle the IRS Rounds switch to ON.

             3. Attempt to submit IRS round entries multiple times (more than 4).

             4. Observe that submissions are accepted beyond the intended limit.

             5. Review the table below — note that the "History" heading is missing.

             6. Observe that rows in the table have no spacing, causing a dense and unclear layout.

      Expected Result:

      • IRS Round submissions should be restricted to a maximum of 4.
      • The table should clearly display a "History" heading.
      • Adequate spacing between rows should be maintained for readability.

      Actual Result:

      • The toggle being ON allows unlimited submissions beyond 4.
      • "History" heading is not shown.
      • Table rows are visually cramped with no spacing.
