Description:
In the application’s submodules—Malaria, Kala Azar, AES/JE, Filaria, and Leprosy—the "Add Member" button does not function as expected.

Steps to Reproduce:

     1. Log into the application.

     2. Navigate to any of the submodules: Malaria, Kala Azar, AES/JE, Filaria, or Leprosy.

     3. to use the "Add Member" button.

     4. Observe that clicking the button yields no response and no new member creation screen appears.

Expected Result:
Clicking the "Add Member" button should open the member creation form, allowing users to add new members to the selected submodule.

Actual Result:
No response upon clicking; unable to add new members in any of the listed submodules. Only add a member to the All Household module.
