As an ASHA I want to report maternal deaths and upload required documents, so that I can contribute to accurate maternal death surveillance and receive incentive for the report. 

Condition: Show MDSR line listing of "Pregnant Women", where in ANC visits the field 'Maternal Death' = "Yes" or in Delivery Outcome section 'Delivery Complication' = "Death" or in PNC section 'Mother Death' = "Yes"

_When maternal Death is reported, should be flagged in the beneficiary detail page._  

Add following fields in the ANC form

Acceptance Criteria:

If any "maternal death" is reported, update 'Beneficiary Status' = "Death" in the Beneficiary record, with “Date of Death ”, "Place of Death" and “Reason for Death” in Beneficiary table and sync to AMRIT

Incentive Eligibility:

• When a maternal death is reported by an ASHA user, the case becomes eligible for an incentive (amount as per scheme).
• Incentive eligibility should be flagged after required documents are uploaded.
• Death Certificate (PDF/JPG/PNG)
• MDSR Form by ANM (Maternal Death Surveillance & Response form)

Upload Specifications:

• Accepted formats: PDF, JPG, PNG
• Maximum file size: [Specify, e.g., 5MB per file]
• Death certificate is mandatory if mother status is "death=yes" before submission.

Validation Rules:

• All mandatory form fields must be filled.
• Upload of all required documents is mandatory.
• Show validation messages for missing or invalid entries.
