As an ASHA, I should able to request for my User account deletion, so that I should not have further login access in to app.
Acceptance criteria:

• There should be a link/ option for the ASHA User to make a request for account deletion
• This option should be available in the App before login in the Login screen
• This account deletion request form should have fields required for identifying the User and Admin to take necessary action
• Form should have fields like Full Name, Username, Contact Number, E-mail Id, State, Project Name, Reason for account deletion
• Once User request is submitted, show status of success or failure
• In success message: "Thank you, User. Your request for account deletion received. Allow us to process your request, we will revert back to you within 30 days, once your account removal process is completed."
• In failure message: Sorry, Please Try Again.
