Description:
After completing and syncing data, multiple modules are showing data loss/mismatch after application reinstallation. The following issues were observed:

1. Malaria – Confirmed Case: Beneficiary is not syncing; Sync Dashboard shows 3 workers failed.
2. Village Level Form: Data is not syncing for AHD, VHND/VHSND, and NDD.
3. Child Immunization: After reinstallation, previously completed forms are appearing again for data entry.
4. ANC: Beneficiary is available in ANC, but the Due for ANC dashboard does not show the data.
5. HRPW Dashboard: Suspected HRPW and Confirmed HRPW counts/data become mismatched or show 0 after reinstallation.
6. High-Risk PW: After completing the Micro Birth Plan, reinstallation again shows the form for data entry.
7. MDSR/Maternal Death: After completing MDSR and marking the pregnant woman as deceased, the data is not reflected under Maternal Death after reinstallation.
8. Child Under 5 Years: Completed SAM, ORS, and IFA forms appear again for data entry after reinstallation.
9. Adolescent: Similar data persistence issue is observed after reinstallation.
