Description:
Multiple issues were observed in the Newborn Registration and HBNC/HBYC Form.

Issue 1: Extra Field Visible in View Mode

After completing the Newborn Registration by filling all the required fields, an additional/extra field is visible while viewing the registration details.

Issue 2: “Is Baby Admitted in SNCU?” Field Issues in HBNC Form

When filling the HBNC Form, multiple issues are observed with the “Is Baby Admitted in SNCU?” field.

Issue Details:

• When Yes is selected, the Discharge field appears incorrectly above the admission-related field.
• After entering/selecting a value, the selected value is not displayed properly.
• When No is selected, the value is also not getting saved/displayed properly.

Issue 3: Incorrect Translation for Breathing Status in HBNC form

For the Breathing field, the translated value was changed from “Fast”, but it is still being displayed as “Fast” in English.

Expected Result:
The Breathing status should display “Normal” instead of “Fast.”

Issue 4: Value Not Displayed in View Mode for Cough/Breathlessness Field in HBYC Form

In the HBYC Form, after entering/selecting a value for the field “Any episode of cough, breathlessness or difficulty in breathing?”, the selected value is not displayed while viewing the form.
