Disease Control enhancements: CBAC Part C exposure option, Malaria PV follow-up to Day 14

      This covers two separate enhancement requests for the Disease Control module, reviewed and approved by the BSA team.

      1. CBAC Part C - Add an "Other sources" option to the exposure/smoke dropdown.
        Today, the COPD-related screening in CBAC Part C does not have an option for exposure sources like second-hand smoke, so these cases fall under "None of these." Add a new dropdown option called "Other sources" to cover exposure that is not already listed - for example, the type of cooking fuel used at home, or occupational exposure at work. When "Other sources" is selected, show a text box where the user can optionally type more detail. This text box is NOT mandatory - the form must still allow submission even if the user leaves it blank.
      2. Malaria -> Confirmed Cases -> Follow-up - Extend PV follow-up tracking from Day 4 to Day 14.
        Today the app only allows tracking PV (Plasmodium Vivax) follow-up visits up to Day 4 after a confirmed case. Extend this so follow-up can be tracked all the way to Day 14.

      Affected screens/modules: Mitanin app - CBAC Part C form (COPD/exposure section), Malaria Confirmed Cases -> Follow-up screen. Backend: CBAC form config (new dropdown option plus optional text field); Malaria follow-up schedule/tracking (extend max day from 4 to 14).
