Description:
In the Maternal Health → ANC → Home Visit module, every home visit is displayed as "Visit 1" instead of incrementing the visit number based on the number of visits completed.

This makes it difficult for users to identify the correct sequence of home visits.

Steps to Reproduce:

1. Login to the FLW application.
2. Navigate to Maternal Health → ANC → Home Visit.
3. Add and save the first home visit.
4. Add a second and subsequent home visits for the same beneficiary.
5. Observe the visit number displayed for each visit.

Actual Result:

• Every home visit is displayed as "Visit 1", regardless of how many visits have already been completed.

Expected Result:

• The visit number should increment correctly for each new home visit (e.g., Visit 1, Visit 2, Visit 3, Visit 4, etc.) based on the beneficiary's visit history.
