Child IFA (6 months-5 years) - Remove visible Bottle Count field, display as IFA Visit N (Mitanin only)

      Current behavior:
      When a Mitanin adds an IFA visit, she enters the Date of Provision. The app auto-increments an "IFA Bottle count" field (n+1) for every monthly visit, and this count is currently visible on the app.

      Required change:
      Remove the "IFA Bottle count" field from the Mitanin-facing UI entirely, it should not be shown or editable by her. The backend continues to count and increment this exactly as it does today (one bottle per monthly visit, already capped at one per month), no change to the counting logic itself.

      In place of the raw count, display each entry as "IFA Visit 1", "IFA Visit 2", etc. as she adds new visits via "Add New Visit". For example, if she gave IFA for the first time last month, it should show as "IFA Visit 1".

      The View option for any visit shows the last date entered, read-only, no edit option, consistent with the existing no-edit rule for Mitanin.

      Scope: Mitanin app only. No change to Utprerona.

      Note: This supersedes the earlier framing of this ticket (tracking distinct months vs. bottle count), no backend counting-logic change is needed, this is a UI simplification only. The separate open question on the incentive's recurring cycle length (6 months vs. the current 10 months) remains on hold pending SHSRC confirmation and is out of scope here.
