Extend the in-app notifications foundation (bell icon, panel, dispatch engine, dispatch log, built in FLW-1082) to the two new roles. No new foundation work expected if FLW-1082 ships first; this ticket only wires up the triggers.

Mitanin Trainer

• On submission/resubmission containing one or more of the four activities: starts her 24 hour window.
• Reminder 6 hours before that window elapses if she hasn't acted.

ANM

• When the Trainer approves the four activities and the claim becomes available for her review.
• Reminder 6 hours before her own due date (5th of month) if still pending.
• Confirmation when she rejects a claim.
• Notification when a Mitanin resubmits following a rejection, once the Trainer has re-approved the four activities.
• Confirmation on her final approval.

Escalation (fallback only)

• SMS/WhatsApp to the Medical Officer mapped to the ANM remains available as a fallback, expected to be rarely triggered if the above notifications are followed.
