[UAT-2.10] Application Crash, Validation Issues, and Irrelevant Fields Displayed in Death Reporting Workflow Across Malaria, Kala-Azar, and AES/JE Modules

      Description:
      Multiple issues have been observed while marking a beneficiary as Death from the Malaria, Kala-Azar, and AES/JE modules. The application crashes during form filling, some fields are not accessible, validations are not working correctly, and irrelevant screening fields are displayed in the Death workflow.

      Steps to Reproduce:

      1. Navigate to Malaria / Kala-Azar / AES-JE module.
      2. Open a beneficiary record.
      3. Select Death as the outcome/status.
      4. Start filling the death-related details.
      5. Select Place of Death and Reason for Death = Other.
      6. Try entering values in dependent fields.
      7. Leave some mandatory fields blank and continue filling the form.
      8. Observe the displayed fields and application behavior.

      Actual Result:

      • Application crashes while filling the Death form in Malaria, Kala-Azar, and AES/JE modules.
      • Fields under Place of Death and Reason for Death = Other are not clickable/editable.
      • Mandatory field validations are not working properly.
      • Users can proceed to the next fields without completing previous mandatory fields.
      • RDT Test and Slide Test fields are displayed even when the beneficiary is marked as Death.
      • Multiple field-level and navigation issues are observed in the Death workflow.
