There is a misalignment issue in the Create Service Point Mapping screen. The following problems are observed:

Fields Alignment: The fields - District, Taluk, Service Point Name, and Descriptions - are appearing in a single line.
Dropdown Arrow Alignment: The dropdown arrow is not aligned properly.
