As a user I want to conduct General OPD (QC) under the visit detail's >> new chief complaint and also to enable refer section in the doctor's module so that doctor can refer a beneficiary for his/her health conditions. 

Acceptance Criteria:

1. Dropdown option to select General OPD (QC) under the visit detail's >> new chief complaint
2. Doctor can add or update beneficiary details.
3. Refer option>>section in the doctor's module
4. Facility Type should be there in the drop down
5. Select facility should work 
6. Submit button should work 
7. Record has to update in DB and show care context link in for the submission of record.
