As a 104 Health helpline user of Bihar, I want to book an appointment on the behalf of beneficiary at the facilities to avail the health services.

Acceptance Criteria: 

1. Consent is captured for the beneficiary 
2. User is able to collect the beneficiary details
3. Calendar is showing with today's date, if current time is ≥ max time of time slots (e.g. if time slot is 2:00-3:00 PM, calendar will show current date to book an appointment)
4. Timeslots are showing on the 104 application **
5. Submit Button, is working 
6. SMS notification shared to Beneficiary with appointment details 
7. Information shared and available at Bhavya's system
