• Ensuring for DPT immunization (5 years). (Assam S. No. 14)
• Assisting in DPT booster vaccination for children aged 5–6 years - (CG: S. No. 36, MIPS Activity Code: 54)

Incentive Logic:
Immunization module for Children Age between 5-6 year,
when a child receives “DPT Booster 2” vaccination,
then ASHA is eligible for this incentive per child in the month of vaccine given date.
 
Follow this below step to test:
Register a child with 5–6 years Age, and fill all above "DPT Booster-2" Vaccine/ as per schedule on due date.
