UAT_TM_MMU_AMM_Prescription fields should be mandatory if user select "form"

      1. Logged in with the valid credentials
      2. Register a beneficiary
      3. Complete the nurse flow
      4. Under doctor module_ prescription
      5. All the field under prescription is non-mandatory but if user is selecting "Form=Tablet/Capsules/Syrup and etc.." then all the field should become mandatory except "Route and Instruction"
