Issue: In History section, if we select 'Risky Sexual Practice Status' as 'No' but appears as 'Yes' in the case sheet

and If we select Allergy- food/Air/Environment, no value comes in the search.

Steps to Produce issue:

1. Login and Register New Beneficiary
2. After Registration, Beneficiary will move in Nurse module
3. In Nurse module, select ‘Risky Sexual Practice' Status as 'No’ in history section (Personal Habits) and submit all the details
4. In Nurse Module- History- Personal history- Risky Sexual Practice and Allergy Type- Food/Air/Environment
5. In doctor module, save all information and click on submit
6. Now, you can see the case sheet. In the case sheet, you can see that ‘Risky Sexual Practice Status’ will appear as ‘Yes’
