Unable to create provider serviceline state mapping in UAT .

      We are unable to create the provider service line–state mapping from Superadmin to create provider admin in the UAT environment, as it displays no record.

      Also attached Screenshot for this.

       

      Steps to be followed -

      • Login as superadmin

      • Click on Provider Master --> Create provider.

      • Click on Activity --> Provider Serviceline-State Mapping --> Create new Mapping --> click on + button you will get blank page. Below are the screen shot.
