Allergy Type, Type of Reaction disappears after moving to doctors screen, also ** Allergy Type, Type of Reaction, and Allergen Name disappear from multiple areas, especially after editing/updating the allergy details or after lab tests are performed.

This results in incomplete allergy information being shown to doctors and missing data in printed case sheets.
