      As an ASHA, I want to submit claims based on contraceptive service delivery, so that I can receive appropriate incentives based on the doses of ANTRA injections administered and other contraceptive delivery services provided.
       

      Acceptance Criteria

      1. Method Selection Logic

      • Default value of Method Selection Dropdown should be null.
      • If "Any Other Method" is selected:
      • Display "Any Other Method" Text Box.
      • The text box should be mandatory for form submission.

       

      2. ANTRA Injection Option Behavior

      • If "ANTRA Injection" is selected:
      • Enable and display the following mandatory fields:
      • Dose: Dropdown with values:
      • 1st Dose
      • 2nd Dose
      • 3rd Dose
      • 4th Dose
      • 5th Dose to 10th Dose
      • Date of ANTRA Injection: Calendar Picker
      • Due Date of Next Injection: Auto-populated or manually entered label
      • MPA Card:
      • Upload File
      • View Image (if already uploaded)
      • ANTRA Injection Details: Display a table summarizing submitted entries

       

      3. Incentive Calculation Logic

      I. ANTRA Injection (Injectable MPA - Antara Program)

      • Incentive Amount: Rs. 100 per dose
      • FMR Code: 3.1.1.2.8
      • Activity Numbers:
      • 1st Dose → S. No. 38
      • 2nd Dose → S. No. 39
      • 3rd Dose → S. No. 40
      • 4th Dose → S. No. 41
      • 5th and above → S. No. 42
      • The selected dose should automatically map to the corresponding S. No. and FMR Code for claim generation.

      II. Social Marketing Incentives

      • Home Delivery of Condoms → Rs. 1 per patient
      • Home Delivery of Oral Pills (Chaya) → Rs. 1 per patient (only if Chaya is selected)
      • Home Delivery of EC (Emergency Contraceptive) → Rs. 2 per patient
