As an ASHA I want to report maternal deaths and upload required documents, so that I can contribute to accurate maternal death surveillance and receive incentive for the report.
Condition: Show MDSR line listing of "Pregnant Women", where in ANC visits the field 'Maternal Death' = "Yes" or in Delivery Outcome section 'Delivery Complication' = "Death" or in PNC section 'Mother Death' = "Yes"
When maternal Death is reported, should be flagged in the beneficiary detail page.
Add following fields in the ANC form

Acceptance Criteria:
Incentive Eligibility:
- When a maternal death is reported by an ASHA user, the case becomes eligible for an incentive (amount as per scheme).
- Incentive eligibility should be flagged after required documents are uploaded.
- Death Certificate (PDF/JPG/PNG)
- MDSR Form by ANM (Maternal Death Surveillance & Response form)
Upload Specifications:
- Accepted formats: PDF, JPG, PNG
- Maximum file size: [Specify, e.g., 5MB per file]
- Death certificate is mandatory if mother status is "death=yes" before submission.
Validation Rules:
- All mandatory form fields must be filled.
- Upload of all required documents is mandatory.
- Show validation messages for missing or invalid entries.
