Implement radio button options under the label "Tele Consultation" for both nurse and doctor user roles.

Add the following radio button options:

• eSanjeevani
• Swymed
• Not Required (Default selection)

Behavior:

• Only one option can be selected at a time (standard radio button behavior).
• The "Not Required" option should be selected by default.

Roles:

The same set of radio buttons should be added separately for:

• Nurse
• Doctor

Acceptance Criteria:

• Radio buttons are present for both nurse and doctor roles.

• "Not Required" is selected by default on initial load.

• Only one option can be selected at a time.
