Throwing below error on nurse submit

Steps to reproduce
Register a benificiary
Proceede with nurse flow with cancer screening service
Enter all the required data in examination screen and click on submit

Expected behaviour
Data saved successfully-prompt should be displayed

Actual result
Throwing An info pop up with mentioned error
