Doctor :History:If selected per week,number field is not getting patched

Steps to reproduce
Register a benificiary
proceede with nurse
in history section under tobacco,select radio button per week and save
Complete the nurse flow
go to doctor
verify past history

Expected behaviour
The details given nurse should get patched
