Issue:
When added more than one row in tobacco, alchohol , allergy,one extra empty row is geting patched

Steps to reproduce
Register a benificiary
proceede with nurse
in history section select more than one row for tobacco,alchohol,and allergy
complete the nurse
Proceede with doctor
Check history tab in doctor

Expected behaviour:
The data given in nurse only should get patched

Actual result
one extra empty row is geting patched
