Description::when clicked on submit and continue, empty info pop up is populating.

Steps to Reproduce:

Log in to the 104 application as a hao
Land the call and complete the hao
In closure screen select valid-valid as call type and subtype and transfer the call to mo
Login with mo,up the respective agent
After call got landed to mo ,avail mo service and got to closure screen
In closure screen celect valid -valid as call type and subtype and click on submit and continue button

Expected Behaviour:
Call should get closed/transfered successfully

Actual Behavior:
Displaying an empty info pp up
