Steps to reproduce
login to mmu application
proceede with nurse flow
give some data in personal history
-select tobacco consuming-yes and select per week radio button, enter the required data and save.
-add multipe type of tobacco use/alcohol/allergy
go to doctor
check personal history

Expected behaviour
All the data that is saved in nurse should get binded here

Actual behaviour
For tobacco if we select perweek -->radio button of per week,and number fields are not binding.
One extra empty row for tobacco /alcohol is displaying
For allergy—allergy name and type of reaction is not binding
