In the Vitals section under the Nurse Module, there is a Personal History (Habits) section with options to record:

Previous Tobacco History
Previous Alcohol History
Previous Allergy History

Users can select Yes or No for Tobacco Use Status, Alcohol Use Status, and Allergic Status during registration. However, after completing the registration and revisiting the Personal History section:

The selected Yes/No values do not reflect unless the specific Type of Tobacco, Type of Alcohol, or Type of Allergy is selected.
This inconsistency is impacting data visibility and accuracy in reports.
Steps to Reproduce:

Navigate to the Nurse Module and access the Vitals section.
In the Personal History (Habits) section, select Yes or No for Tobacco Use Status, Alcohol Use Status, and Allergic Status.
Complete the registration process.
Revisit the same record and check the Personal History (Habits) section.
